If any of your personal and/or health insurance information has changed please download, print and fill out out the following form and bring it with you to your next visit. If you have a new health insurance card, please bring it in so we can make a copy.
If you have any questions we would be happy to assist you on the day of your appointment.
If you are unable to make your appointment, please give us 24 hours advanced notice.
Missed appointments, with less than 24 business hours notice, prevent us from meeting your immediate healthcare needs and limits our office’s ability to provide rehabilitation services to those who would otherwise utilize your appointment time. If you have a conflict with your scheduled time, we would greatly appreciate a phone call more than 24 hours in advance of your appointment time to reschedule.
We appreciate the fact that your time is as valuable as ours. We make every effort to see you as close to your scheduled appointment time as possible. Please assist us in doing so by arriving 5-10 minutes prior to your scheduled time. For patients arriving more than 30 minutes after their appointment time, we will do everything we can to still see you, however, you may be asked to reschedule.
Fees & Payment
If we are a participating provider with your insurance company we will bill them. Any deductibles, co-pays, and co-insurance are your responsibility. If you have any questions regarding health insurance and the payment system we would be happy to assist you.
If you decide to pay for services in cash, we would be happy to work with you in regards to setting up a payment plan. We accept cash and check.
